CASE MANAGER - STOUFFVILLE
1 Full-Time Position, 1 year contract with possibility of extension


DATE: May 17, 2018

LOCATION: Stouffville, Ontario


JOB SUMMARY

The transitional housing assisted living hub will provide transitional support and housing to individuals living with serious

mental health and addiction challenges, referred through hospitals as a “flow option” from their ED or ALC beds or an

alternative to hospitalization. The Case Manager will provide a range of service to support the individuals in reintegrating

into more independent settings.The goal is to increase their independence through skill enhancement and skill

development, by building a support network and by linking individuals to available community services. The case

manager also provides needs assessment (OCAN), coaching, skills teaching, advocacy, crisis management, facilitating

house meetings and/or psycho-educational groups and support with daily living. This position liaises with other

community agencies and professional services. The case manager is required to share on-call responsibilities on a

rotational basis for all LOFT/Crosslinks programs. Travel within CLHIN catchment area and occasionally to the

downtown Toronto office location is an expectation of the position.

Note: Will be required to work with individuals in their rooms and may therefore be exposed to second hand smoke.


QUALIFICATIONS 

Completion of a degree/diploma in Social Work/Human Services or equivalent; 

Minimum of three years related experience working with the target population; 

Ability to assist individuals with daily living activities, promoting the acquisition of life skills; 

Understanding of psychosocial rehabilitation and recovery principles, harm reduction, and the therapeutic relationship and
the ability to apply these concepts into practice; 

Knowledge and ability to assist in the achievement of the fundamental components of recovery; 

Knowledge of issues faced by adults with serious mental illness and of available community and other professional
services in CLHIN catchment area 

To support individuals to define their own life goals and unique path to achieve those goals; 

Good oral and written communication skills, including ability to write timely and accurate client notes within professional
standards; 

Ability to work effectively within a team environment and to interact with workers from a variety of agencies; 

Crisis intervention skills, with well developed problem solving abilities; 

Previous experience in supportive housing is an asset; 

Ability to assist in cleaning and moving inventory to prepare units for occupancy; 

Computer literacy; 

Possession of a valid driver’s license, use of a reliable vehicle, and insurance to transport clients is required
