Community Support Worker Jobs Vacancy in Canadian Mental Health Association Peel Branch Brampton Jobs Details:
The Canadian Mental Health Association/Peel Dufferin Branch is recruiting for a Community Support Worker- Housing to work with clients of the Housing and Support Peel (HASP) program.
The incumbent will link with and assist persons with serious mental illness who are homeless or at risk of homelessness and who are eligible for referral to Phase II funded units from Supportive Housing in the Province (SHIP); once the client is housed, the worker will provide the necessary community support in order to maintain housing.
This is a 6 month contract position of 37.5 hours per week and the incumbent reports to the Manager –Mental Health & Justice Services/HASP teams.
Salary band $49,500 - $59,500 per year
To provide case management services a case load of clients.
To assess the clients need for supportive housing, and their eligibility for access to units provided within the Housing and Support Peel Program.
To facilitate the client’s move into their supportive housing unit, as needed.
To establish appropriate rehabilitation goals and assist the client in reaching them
To provide crisis intervention, when necessary
To refer clients to appropriate programs
To provide ongoing supportive counseling and case management, when appropriate
To provide support specific to life skills, daily living skills, conflict resolution, crisis intervention and prevention, budgeting assistance, community integration and community building
To conduct case conferences/consultation with treatment providers, including physicians, other community organizations and other significant persons, as deemed appropriate
To respectfully advocate on behalf of clients
To maintain current knowledge of various linkages with community resources.
To maintain effective and timely documentation in client records
To attend regularly scheduled team meetings
To participate in regular supervision including annual performance appraisal with the Program Manager
Follow PHIPA legislation and CMHA Peel Dufferin policies and procedures governing Privacy and client personal health information and health records.
To participate as a full member of HASP team.
To provide the court support function to clients as directed.
To complete other duties as assigned
Skills & Qualifications:
University degree or College Diploma in Social Work or Social Services. BSW preferred.
Regulated Health Professional designation is an asset.
At least 3 years of relevant case management experience is required in a community mental health setting.
Working knowledge of the Mental Health System in Ontario and Recovery principles.
Demonstrated ability to perform risk, need and psychosocial assessments.
Demonstrated ability to work independently in a multi-task environment and to prioritize tasks in a manner that meets client needs and program objectives.
Ability to communicate in both verbal and written form with professionals from a
variety of disciplines (i.e. medical, legal).
Knowledge of CRMS and OCAN is an asset.
Training in ASIST, Non-Violent Crisis Intervention and Crisis Response is an asset.
Knowledge of French or any other second language that reflects the diversity of Peel Region is an asset.
A satisfactory Vulnerable Sector Screening (Police Check) report is required.
Current CPR/First Aid certification.
Current driver’s license, relevant insurance and access to a reliable vehicle.
