Contract Fulltime Mental Health Access Facilitator

Current employee on leave - Contract starting immediately and ending October 2018

Key duties and responsibilities

  • Provide support to individuals with a dual diagnosis, and their family members and staff, to identify resources and connect to services
  • Prepare and present networking and learning opportunities (training, workshops, seminars, etc.) for medical professionals, workers in community services, and NHCIA staff
  • Provide support by: arranging psychiatric and/or individualized multidisciplinary assessments; developing treatment plans in consultation with behavioral therapist or other professionals; monitoring treatment responses and conducting treatment/program evaluations; making referrals to service providers; and being available for emergency support staffing on an individualized basis
  • Act as a liaison to the ACT Team, Psycho-Geriatric Outreach Team, DDCOT, Children and Adult Mental Health Services, Behavioral Consultants, DSO, Probation, OPP and other legal services, APSW, and other resources to promote mental health
  • Ensure access to mental health/dual diagnosis services is enhanced and individuals understand their right to receive these services
  • Provide a link and informationto local and regional dual diagnosis committees, pressures and priorities, and other support services
  • Provide direction and support to individualized support worker

  • Knowledge of CPI nonviolent crisis intervention and ability to utilize these practices
  • Ability to record documentation and collect and compile data
  • Skill using computers, technology, and Microsoft Office software including Outlook, Word, and Excel
  • Commitment to maintaining a high standard of attendance and punctuality

Requirements

  • Successful completion of the Developmental Service Worker or Social Service Worker diploma or an equivalent combination of education, training, and experience
  • Experience working with people with a dual diagnosis and developmental disability
  • Current First Aid and CPR
  • Successful completion of a pharmacology course at the college level. This may be completed during the first 6 months
  • A valid G class driver's license, clean driving record, reliable vehicle, 6a endorsement, and $1,000,000 liability coverage
  • Clean criminal record check including vulnerable sector screening
  • Ability and willingness to work flexible hours including evenings and weekends
