Physician Assistant Jobs Vacancy in Humber River Fht Toronto Jobs Details:

The Humber River Family Health Team (HRFHT), a growing, multi-site FHT that provides collaborative, interdisciplinary, comprehensive primary health care to a rostered population of patients in the northwest Greater Toronto Area (GTA) with a range of complex healthcare needs and very diverse backgrounds. The FHT is committed to holistic, compassionate, patient-centred care emphasizing illness prevention, chronic disease management, harm reduction and health promotion.

HRFHT seeks a Physician Assistant (PA) to provide medical care to patients based on mutually agreed upon guidelines, while working under the supervision of a registered physician and with a team of health professionals in an inter-professional collaborative primary care setting. The PA scope of practice is unique from that of other health care disciplines in that the PA is not an autonomous practitioner. All tasks must be delegated following a recognized process of delegation to the PA by the supervising physician. The type of work delegated, and the extent of direct supervision provide to the PA, is dependent on the physician’s assessment of the PA’s individual competencies, skills and experience in that practice setting. Further, only work within the physician’s own scope of practice can be delegated to a PA.

The PA cannot act as the principle medical decision maker and reports to the physician supervisor for all clinical matters. The PA collaborates with physicians and other FHT members to provide the most efficient, appropriate primary care to a patient.

Qualifications:
Graduate of accredited Physician Assistant program (McMaster or Consortium)

Current registration in good standing with the Canadian Association of Physician Assistants(CAPA) is required

Valid Ontario Drivers’ License and access to own method of reliable transportation.

Demonstrated experience in community or primary health care setting.

Strong interest and experience in program development, implementation and evaluation of health promotion, disease prevention and chronic disease management

Able to function as a team player working collaboratively with other FHTs staff and community partners

Excellent communication and problem solving skills

Sound organization, communication, teaching and leadership skills

Proficiency in the use of computer and communication system knowledge- email, EMR

Knowledge and proficiency in current, evidence-based methods and practices of primary care delivery, with an emphasis on health promotion and risk reduction.

Two years’ experience in a primary care setting preferred

Position Responsibilities :
Knowledge and proficiency in current, evidence-based methods and practices of primary care delivery, with an emphasis on health promotion and risk reduction.

Complete health assessments including health history and physical examinations within delegated acts

Provide wellness care and health screening

Screen for the presence of chronic illness and monitor stable chronic illnesses

In collaboration with the patient, physician and other team members identify priorities, develop and implement plans of care

Patient centred approach that empowers patients to take greater responsibility for their own health.

Assessment, education, and self-management goal setting with chronic diseases for individual patients.

Develops and maintains an understanding of services, programs and resources available within the FHT’s and in the community in order to refer the patient and reduce duplication and improve timeliness of service delivery.

Evaluates patient results after a treatment plan is implemented.

Participate with other FHT team members on QIP or quality improvement initiatives aimed at improving the health of patients.

Work closely with team members and Executive Director to monitor and evaluate services and programs. Provide the statistics specific to the work of the PA for the quarterly reports due to the MOHLTC.

Participates in self-directed learning to ensure that his/her practice remains relevant by attending professional conferences, e-learning and journal reviews.

Maintain professional competence and remain current (e.g. evidenced-based care) through ongoing participation in professional development, FHT research projects, etc.

Superior multi-tasking skills and organizational ability to prioritize a wide range of job activities with accuracy and speed for delivery of efficient primary care

Communication skills which reflect the ability to work well in a team environment while maintaining respect and attentiveness to patients and fellow team members

Interpersonal skills to listen effectively and to train and help develop patients, new employees and students

Problem solving skills to identify and effectively solve problems and to help patients and team members solve problems

Time management abilities to effectively set priorities and complete tasks within agreed upon timeframes
